Dental Bridges: A Fixed Solution for Missing Teeth

A dental bridge has been a foundational restorative option for over a century, and remains an appropriate solution for many patients today. When a tooth is missing and the adjacent teeth are already planned for crown restoration — or when implant surgery is not the right option — a conventional bridge provides a fixed, non-removable replacement that restores both function and aesthetics in approximately two appointments.

The procedure involves preparing the two teeth on either side of the gap (the abutment teeth) to receive crowns, taking precise impressions or digital scans of the prepared teeth and the opposing bite, and fabricating the bridge at a dental laboratory. A temporary bridge protects the prepared teeth during the 2–3 week fabrication period. At the second appointment, the final bridge is tried in, adjusted for fit and bite, and permanently cemented.

The design of the bridge — the shape of the pontic (artificial tooth), the contact between the pontic and the gum tissue (the pontic design), the emergence of the abutment crowns, and the occlusal contacts — requires the same level of care as any complex restoration. Poorly designed pontics trap food and cause gum inflammation. Poorly calibrated occlusal contacts cause premature wear and fracture. Maryland Bridge (Resin-Bonded Bridge)

A Maryland bridge — also called a resin-bonded bridge — is a conservative alternative to a traditional bridge. Instead of preparing (grinding) the adjacent teeth for full crowns, a Maryland bridge uses thin metal or ceramic wings bonded to the backs of the neighboring teeth. A false tooth (pontic) is suspended between these wings to fill the gap.

Maryland bridges are best suited for replacing a single missing front tooth when the adjacent teeth are healthy and unrestored. The primary advantage is preservation of tooth structure: no grinding of healthy teeth is required. The main limitation is that the bonded wings can debond over time, particularly in areas of heavy bite force. When indicated, a Maryland bridge offers a minimally invasive fixed option that can last many years with proper care.
